ERIC & Deloitte Wellness Program Survey

Survey Close Date: Thursday, July 16, 2005

The ERIC Health Policy Committee and Deloitte have launched a Wellness Program Survey. This short survey has only 17 questions (15 are yes/no or multiple choice questions). The results of the survey will be discussed by the Honorable Tommy Thompson during his keynote address at the ERIC Major Employers' Forum on Controlling Your Healthcare Budget on June 22, 2005 in Washington, DC. Please take a few minutes to complete this survey so that we will have a 100% participation. ERIC staff will contact member companies that have not responded by the close date and request their participation. Survey results will remain strictly anonymous and no individual company will be identified when survey results are released. The link below will take you to the survey:

The survey is not limited to one person from each company. We encourage numerous participants from each employer to complete the survey. ERIC and Deloitte staff will control for duplicate member responses before the results are released but having additional input is desirable.
