Joint Principles of the Patient Centered Medical Home

Below please find the Joint Principles of the Patient Centered Medical Home. These Principles were drafted and agreed upon by the four Primary Care physician organizations - American College of Physicians, American Academy of Family Physicians, American Osteopathic Association, and American Academy of Pediatrics.

This document forms the core of all the work that the Collaborative has been undertaking to support the Medical Home concept.
